GHOTES 2005 Registration Form

1.  Complete the form by typing the requested information in each box. Click on the "Submit" button. This will send the form and add your name to the list of participants.

2.  We aren't capable of taking credit card orders, so you'll need to send a check. Make the check payable to "GHOTES Conference" and mail it to: Betsey Norgard, 1556 Hillview Rd., Shoreview, MN 55126. Important: Please enclose a copy of this form (completed) or a note with a reminder of what you are requesting.
